Hi, everyone. I’m Patrice and today I’d like to talk to you about what it means to apply for your CLIA certificate. Now, many people don’t understand what CLIA stands for, it’s actually the Clinical Laboratory Improvement Amendments. They’re federally regulated standards that apply to any and all clinical laboratory testing that’s performed on humans in the United States. Now, the first thing you have to do is determine whether or not your center is going to perform waived or moderate testing.
Waived testing is probably the most common and the one that you’re probably most familiar with. That usually covers things like rapid strep, rapid flu, urine pregnancy, urinalysis and those types of things that are most commonly done in urgent care. To begin, you’re going to want to fill out what we call the CMS-116 form and that can be downloaded right off the web. Now, the only problem is, you have to know what your state regulation is. Most states will have just that CMS-116 form. However, certain states, and there are a few, that will include additional forms and requirements even to and including, inspections—and that depends on the level of complexity that you choose or the state that you live in—so you’ll have to do a little bit of research.
